As Flu Shot Distribution Nears, Learn About the New Platform for Employees to Complete Required Forms

Methodist believes in the importance of vaccination, which is why we will once again be offering free flu shots for employees at various locations throughout the health system beginning Sept. 25.

To streamline the flu shot process for staff, Methodist has added consent and declination forms to a new platform called ReadySet. You can save time by completing the following steps before arriving for your flu shot.

Simply sign in to ReadySet and look for the Seasonal Flu Immunization Survey under the Health Surveys page. If prompted to log in, be sure to include @nmhs.org at the end of your username, then use your network password.

You will select one of the following options: 

• I AGREE to receive the seasonal flu vaccination
• I ALREADY RECEIVED the seasonal flu vaccination and I can provide documentation
• I DO NOT WANT the seasonal flu vaccination

Make your selection, complete the questionnaire, then click "Submit." For more information, including detailed instructions, please review this knowledge article. You can also find this information by searching “ReadySet” in the People Portal.

Per Methodist's Immunization Programs policy, a consent form, written proof or declination form must be completed by all employees/volunteers in ReadySet by Nov. 21, 2023.
