2016 Flex Plan Claim Filing Deadline: March 31

This is a reminder for employees’ 2016 Flex Plan claims for Medical Expense Reimbursement Plan or Dependent Care Expense Reimbursement Plan. 

• Any Flex claim incurred in 2016 must be received by Discovery Benefits by March 31, 2017. 

• 2016 claims received after March 31 will not be eligible for reimbursement, and plan balances will be forfeited.

You can find the Flex Plan Claim Forms on mhsintranet under the “Forms” tab.  Use the Search for field and type “Flex Reimbursement Form”.
