Get a preventive exam by Aug. 31 to secure your health insurance wellness incentive

Preventive visit, wellness exam, routine exam, annual physical – all of these terms refer to the same thing: a yearly check-in with a provider to stay on top of your health. 

Get an annual or routine preventive exam with your primary care provider or gynecologist between Sept. 1, 2023, and Aug. 31, 2024, to receive the wellness incentive in 2025.

Getting a preventive exam results in the following premium savings for Methodist Health System (MHS) Health Care Plan participants:

Requirements

  1. The employee and/or spouse must be covered on the MHS Health Care Plan as of Aug. 1, 2024.
  2. Complete your annual preventive care visit between Sept. 1, 2023 and Aug. 31, 2024.
    1. Qualifying provider types: family medicine, internal medicine or OB/GYN
    2. Qualifying provider level: physician, physician assistant (PA), nurse practitioner (NP) or certified nurse midwife (CNM)
  3. An appropriate preventive care office visit code (CPT) must be billed to qualify. Preventive lab only does not qualify for the incentive.

Quick tip: Take a copy of this information to your visit and share with your provider so they can understand the wellness incentive program.

Note: The MHS Health Care Plan covers one preventive wellness visit per calendar year. You don't need to have more than 365 days between visits. If it's unreasonably difficult to complete a routine preventive exam due to a medical condition or being medically inadvisable, contact Kelli Petersen.
