Get the flu shot at one of these locations, and lunch is on us

According to the Centers for Disease Control and Prevention, getting the flu vaccine has many important benefits. It can keep you from getting sick with the flu, or make symptoms less severe if you do become sick. In turn, the vaccine helps reduce days of missed work and school, visits to doctor’s offices, and flu-related hospitalizations and deaths.
Methodist believes in the importance of vaccination, which is why we will once again be offering free flu shots for employees at various locations throughout the health system. Lunch vouchers will be provided to participating employees.
Save the date for one of the following flu shot locations:

Consent and declination forms
To streamline the flu shot process for staff, Methodist has consent and declination forms in a platform called ReadySet. You can save time by completing the following steps before arriving for your flu shot.
Simply sign in to ReadySet and look for the Seasonal Flu Immunization Survey under the Health Surveys page. If prompted to log in, be sure to include @nmhs.org at the end of your username, then use your network password.
You will select one of the following options:
- I AGREE to receive the seasonal flu vaccination
- I ALREADY RECEIVED the seasonal flu vaccination and I can provide documentation
- I DO NOT WANT the seasonal flu vaccination
Make your selection, complete the questionnaire, then click "Submit." For more information, including detailed instructions, please review this knowledge article. You can also find this information by searching “ReadySet” in the People Portal.
Per Methodist's Immunization Programs policy, a consent form, written proof or declination form must be completed by all employees/volunteers in ReadySet by Nov. 21, 2024.
